Minor Release Forms
All minors must have a legal
guardian PRESENT
at registration to sign-up for practice and racing.
If the legal guardian will
not be present at registration, the minor must be accompanied by an adult
designated by the legal guardian(s) with the following forms SIGNED
and NOTARIZED. You will need to complete one form for
each event. For example, you would
need one copy of each form for a Saturday practice, and one copy of each form
for the race on the following day.
The following forms must be printed in COLOR, completed, notarized,
and present at registration if the legal guardian(s) will not be
present:

Release Form
This form must be SIGNED
and NOTARIZED
by the legal guardian(s) if not present at registration.
Minor
Release Form
BOTH PAGES of this form must printed in COLOR, SIGNED, and NOTARIZED by the legal guardian(s) if not present
at race registration.
