Minor Release Forms
All minors must have a legal guardian PRESENT at registration to sign-up for practice and racing.
If the legal guardian will not be present at registration, the minor must have completed the Competition Annual Minor Liability Release. Once this is completed and returned to the AMA, an Annual Minor Liability Release card will be issued. If the legal guardian(s) will not be present, then either this card must be present or the below form must be fully completed by all legal guardian(s), notarized, and presented at registration.
